What warnings and precautions should i be aware of before starting finasteride?

Prostate Cancer:

  • There may be an increased risk of a more serious form of prostate cancer in men taking finasteride. In one clinical trial patients taking 5mg finasteride had an increased risk of being diagnosed with a more serious form of prostate cancer (high-grade prostate cancer). If you have a history of prostate cancer or are at high risk, it is advised that you discuss finasteride with your urologist/oncologist before starting treatment
  • If you have a history of urological problems such as benign prostatic hyperplasia (BPH) with intermittent urinary tract obstruction, a healthcare provider may need to monitor you more closely because these symptoms can overlap with signs/symptoms or prostate cancer

Prostate Specific Antigen (PSA): Finasteride can affect the levels of a substance called PSA in your bloodstream, which is used as a marker to detect or monitor prostate cancer. It can decrease your PSA levels which has the potential to mask underlying prostate cancer. Any changes from your baseline PSA can indicate a possible prostate cancer, even if your levels are within normal limits. Therefore it is important that you inform any provider

Pregnancy: Finasteride is a Category X teratogenic medication that can cause birth defects in male fetuses. Women who are actively planning to get pregnant, are pregnant or breastfeeding should not should not use finasteride and should not touch broken tablets or liquid.

Fertility: There is some evidence that finasteride may affect sperm counts/quality and can be secreted in the sperm in very low amounts, but currently there are no recommendations based on data from clinical trials to suggest that men should avoid taking Finasteride during conception or while a partner is pregnant.

Blood Donations: Do not donate blood while you are taking this Finasteride. Ask your doctor or health care professional when it is safe to donate blood after you stop taking this medicine.

Mental Health: Finasteride has the potential to trigger psychological side effects like depression, anxiety, brain fog and there have been rare reports of suicidal thoughts. If you have a history of severe depression or another mental health condition, it is advised to consult with a mental health professional prior to starting therapy.

Drug interactions:

  • Before starting treatment with finasteride, let your healthcare provider know about any medications or supplements you’re taking
  • Combining finasteride with other medications/supplements that have the same mechanism of action can increase the risk for side effects and should be done with caution and under the advisement of a healthcare provider. Finasteride, Dutasteride and a dietary supplement known as Saw Palmetto (Serenoa repens) have similar effects in the body. These can be used for hair loss and in people with benign prostatic hyperplasia (BPH).

Side effects

Serious side effects from finasteride when taken as directed are typically rare. There is some evidence that finasteride can increase the risk of breast cancer or prostate cancer.

The following have been reported with finasteride use:

  • Breast tenderness and enlargement. Tell your healthcare provider about any changes in your breasts such as lumps, pain or nipple discharge
  • Depression, anxiety, brain fog, suicidal ideation
  • Decrease in sex drive that continued after stopping the medication
  • Allergic reactions including rash, itching, hives and swelling of the lips, tongue, throat, and face
  • Problems with ejaculation that continued after stopping medication
  • Testicular pain
  • Difficulty in achieving an erection that continued after stopping the medication
  • Male infertility and/or poor quality of semen
  • In rare cases, male breast cancer

Please note that finasteride has only been formally studied in male patients so the side effect profile above is specific to clinical studies in men.

When used off-label in a topical form, systemic side effects of finasteride are expected to be less frequent/severe, although no formal clinical studies have been done to confirm the safety and efficacy of topical finasteride.
